Magnolia’s Survey Disclosure Policy
Your response to this survey is voluntary. Your responses will be used only for the purposes outlined at the beginning of the survey. Participation in this survey is by invitation only - please do not forward this survey. Please respond only once. Participation in this survey indicates your acceptance of the terms below.
Privacy and Data Usage Policy
This Privacy and Data Usage Policy applies to information that can be used to identify you (“Personal Information”) and that you provide to us or which is derived from the Personal Information you provide, as outlined below. We will use our survey provider cookies to assure the smooth completion of your survey.
What information do we collect?
We collect Personal Information provided by you such as name and email address and your responses to the survey questions. We may also obtain additional Personal Information about you from third parties and will treat this additional Personal Information according to this Privacy Policy, plus any additional restrictions imposed by the third party that provided us with it or applicable law. The survey software company we use, Qualtrics may collect IP addresses from survey respondents to prevent and protect against fraud and malicious activity, and to ensure the security of the website, app or cloud service of Qualtrics.
What we do with your information:
Data collected as part of your survey responses is analyzed and used to suggest improvements to the program that is the subject of the survey and in other research. We may follow up with you by email, text, or telephone to clarify your survey responses or request additional information . We also may share your Personal Information and survey responses with third-parties for the purposes of research, analysis, demographic profiling, and program improvement. Finally, we may use your Personal Information ourselves for follow-up about additional surveys that may be of interest to you or other services or products that may be of interest to you. We will never sell your Personal Information . We never use or share Personal Information provided to us in ways unrelated to the ones described above unless specifically agreed to by you or to comply with legal requirements, such as a law, regulation, search warrant, subpoena, or court order.

How we store your information:
To prevent unauthorized access, maintain data accuracy, and ensure the correct use of information, we have put in place appropriate physical, electronic, and managerial procedures to safeguard and secure the information we collect.
How long we keep your information:
We will only store your Personal Information for as long as it is reasonably necessary for the purposes we’ve outlined above, or as required by law.
Our requirements of third parties with whom we may share your data:
We require all third parties to comply with a privacy policy that is as good or better than ours.
Children’s Online Privacy Protection Act:
This survey is not directed to persons under the age of 13. We do not knowingly collect Personal Information from children under the age of 13. If you are a parent or guardian and believe that we have collected information from a child without your consent, contact us at magnolia.general@magnoliaimpact.org and we will delete such information from our files.
Your rights:
Right to know: You can request that we disclose to you: (1) the categories and/or specific pieces of Personal Information we have collected about you, (2) the categories of sources for that Personal Information, (3) the purposes for which we use that information, (4) the categories of third parties with whom we disclose the information, and (5) the categories of information that we disclose to third parties. You can make a request to know up to twice a year, free of charge.
Right to delete: You can request that we delete Personal Information we collected from you and tell our service providers to do the same, subject to certain exceptions (such as if the business is legally required to keep the information).
Right to correct: You may ask us to correct inaccurate information that we have about you.
Right to opt-out of sharing: You may request that we stop sharing your Personal Information (“opt-out”), including via a user-enabled global privacy control. We cannot share your Personal Information after we receive your opt-out request unless you later provide authorization allowing us to do so again.
Right to limit use and disclosure of sensitive Personal Information: We do not collect Sensitive Personal Information such as financial account numbers, your precise geolocation data, or any genetic or health information data. In the event that we do collect such information, you can direct us to only use your Sensitive Personal Information for limited purposes.
Right to non-discrimination: We will never provide a different level of services to you just because you exercised your rights under this policy. However, if you refuse to provide your Personal Information or ask us to delete or stop sharing your Personal Information , it will limit our ability to include your data in any kind of analysis and suggested program improvements, and we will not be able to follow-up about additional surveys that may be of interest to you or other services or products that may be of interest to you.
